POSITION PURPOSE AND SUMMARY

Under the general supervision of the Supervisor of Cardiac Rehabilitation provides quality rehabilitative nursing care to cardiovascular patients (and pulmonary patients) in the inpatient and outpatient phases of rehab.  Uses the nursing process in providing that care, including effective assessment, planning, implementation, and evaluation skills.  Demonstrates sound judgment based upon knowledge and application of professional, departmental, and organizational principles and standards.  Educates patients in specified areas of cardiopulmonary rehabilitation and other related areas (i.e., cardiovascular anatomy, exercise, risk factor modifications) and maintains the patient’s records as well
EDUCATION/EXPERIENCE:

  • Graduate of an accredited school of nursing.
  • Current professional licensure by the State of Indiana Health Professions Bureau.
  • BSN preferred.
  • Previous CV experience of at least 1 year (CCU, ICU, ED, CVU, IMCU).
  • Previous cardiac rehabilitation experience preferred.
  • Demonstrated ability in arrhythmia identification and intervention, 12 lead EKG interpretation.
  • Obtains BLS and ACLS certification within 30 days of hire and maintains current certification thereafter.
  • Demonstrated ability to control personal risk factors.
  • Successful completion of the Cardiac Rehabilitation orientation programs.
  • Successful completion of the Silver Sneakers group class certification.
  • Computer skills helpful.

    1. Professional Training
-Assumes responsibility for attending mandatory skills validation, certifications, and CE requirements as designated by unit standards and hospital policy.
-Maintains professional licensure.
-Demonstrates knowledge and skills necessary to provide care appropriate to the ages of the patients served on assigned unit.
    1. Emergency Training
-Obtains and maintains current CPR and ACLS certification.
-Functions as a team leader in emergency medical treatment should patient present in any stage of cardiopulmonary arrest and initiates emergency interventions including BLS, AED use, and defibrillation (if hospital policy allows).
-Has access to storage areas that contain medications and/or supplies to include IV fluids/sharps/syringes. Takes part in department/hospital procedures for maintaining crash/emergency cart.
    1. Phase 1
-A progressive exercise/activity program for the Phase I patient is initiated while monitoring EKG and assessing hemodynamic status.
-Makes appropriate recommendations for ambulation when the patient will not be directly supervised by the Cardiac Rehab staff.
-Implements individualized patient plan of care.
-Documents the patient response and effectiveness of nursing interventions on the patient’s medical chart.
-Assures continuity of care by communicating with appropriate staff regarding patient problems/needs.
-Educates the patient and family about CAD, MI, healing, post-operative recovery, signs and symptoms of problems, home activity guidelines, and smoke cessation.
    1. Phase 2/Phase 3: Patient Orientation
-Assesses and screens patients referred to the program.
-Orients the patient to the exercise program. Obtains medical history by reviewing chart and interviewing patient. Performs brief physical assessment. Instructs the patient in the proper use of each piece of exercise equipment.
-Assessment of patient’s needs, design a progressive exercise/activity program, develop appropriate interdisciplinary treatment plans.
-Explains patient consent forms and assures that they are signed. Documents findings in patient’s medical chart.
    1. Phase 2/Phase 3: Patient Education
-Educate patient and family about CAD, principles of conditioning, exercise, diet, lipid management, medications, risk factor modification, and smoke cessation.
-Utilizes a variety of teaching techniques appropriate to each patient.
    1. Phase 2/Phase 3: Patient Counseling
-Assists the patient in formulating and achieving goals during the rehabilitative period, altering goals and interventions as needed.
-Applies motivational counseling and teaching to elicit positive lifestyle changes.
-Functions as disease manager/case manager for a designated group of patients.
    1. Phase 2/Phase 3: Patient Exercise
-Guides the patient through a safe, progressive program of exercise therapy by developing an individualized exercise program based on stress test and exercise prescription in conjunction with program policies and/or Medical Director or attending physician recommendations.
-Educates the patient on correct body mechanics during exercise routine (warm-ups/cool-downs, muscular strengthening, exercise equipment, mounting and dismounting).
    1. Phase 2/Phase 3: Patient Care Interventions
-Monitors and documents patient’s response to each exercise session.
-Documents unusual or adverse patient behavior during exercise and nursing interventions.
-Communicates patient problems to the physician.
-Processes medical/physician orders by department policies.
-Prepares progress notes to physicians.
