ASSISTED LIVING
Assisted Living is ideal for individuals who require some help with life's everyday tasks. The Hartsfield Village Assisted Living apartments licensed by the State of Indiana have nursing presence 24 hours per day, 7 days per week, to assist with the activities of daily living.  This is provided in a residential setting that is beautiful to the eye as well as the spirit.  
SCHEDULE:
Primarily M-F during business hours. Requires occasional overtime or hours outside the normally defined workday or workweek. 
WHAT YOU WILL DO:
  • Identify resident services issues and needs and counsel the residents and families, resulting in positive resolutions and experiences
  • Support residents and families during times of need, crisis, and bereavement and provide assistance to residents, advisors, and families throughout residency.
  • Assist with compliance of all state and federal laws regarding the operation of a licensed residential community by communicating nursing and health care needs of residents to appropriate management and staff
  • Develop and maintain positive relations with community resources and vendors in order to obtain optimal census levels and retain existing IL and AL residents.
  • Coordinate admission process for Assisted Living and Independent Living
  • Promote constructive working relationships between health care disciplines throughout all levels of care.
  • Assure that residents rights are maintained at all times, reporting suspected deviations to supervisor and attend management meetings as required
  • Meet both internally with Hartsfield Village RCNC discharge planners and social workers, and externally with referral hospitals and care providers as needed to assist in facilitating new resident move-in or to obtain needed services for existing residents to advise the referral source of new community programs and services
  • Serve as a professional resource to healthcare system and surrounding community with various programs and services including Community Memory Screenings, Alzheimer’s and Dementia Support Group, and educational presentations
  • Conduct pre-admission assessments for Senior Living prospects (IL, AL, and MS) and provide feedback and recommendations for appropriate level of living to the Senior Living admissions team
  • Conduct assessments for current Senior Living residents who may be returning to, or transitioning to, various levels of care on campus, conduct annual assessments for all Independent Living residents
  • Maintain organized and accurate files, monitor and track caregivers for IL residents, and manage the Resident Observation Forms (ROF) process
  • Participate in admissions meetings with both the Level of Care Team and the Residential Care Administrator to assist in the approval of pending admissions. Provide recommendations as to the appropriate placement, time, and date of transfer and confirmation that specific equipment or service needs of new AL resident have been provided 
REQUIREMENTS:
  • Degree in Social Work preferred
  • Minimum two years’ experience in retirement community field
  • Preferred experience in the areas of social work, marketing, sales, move-in coordination and resident services
  • Knowledge of all phases of the operation of a continuing care retirement community, including housekeeping, dietary, maintenance, and nursing.
  • Ability to communicate with empathy and respect, to analyze issues and identify practical solutions
  •  Understand the needs of seniors and remain aware of trends in Senior Living.
  • Must have knowledge of community agencies and other resources for making referrals for family problems as well as the residents’ direct personal needs.
  • Must be able to plan, implement, and work with only minimal direct supervision.
  • Must be a good listener and be able to display understanding and helpful attitude toward the problems and concerns of the resident population.
  • Proficiency in the utilization of appropriate records including Matrix Care, EPIC, and Matrix Care Marketing (REPS).
  • Basic business computer knowledge preferred.
