Position: Clinical Case Manager - Clinic
Location: East Chicago, IN
Job Summary:
Reporting to the Clinical Director of Occupational Health, ensures efficient and effective clinic operations at all occupational health locations through focused and goal-oriented clinical leadership.  Supports the medical providers by overseeing and providing case management services for the occupational health patients and through coordination of case management services for all locations.  Assist with the development, implementation and management of a patient care that efficiently, effectively, and proactively manages clinical and quality outcomes.  Provision and promotion of quality care for clients and patients through the utilization of an interdisciplinary team approach.
 
Education/ Experience Requirements:
•       Bachelor's Degree required; Master's Degree preferred
•       Indiana RN License required
•       CPR certification required
•       Maintain license and/or certification per state guidelines
•       Previous management/supervisory experience preferred
•       At least 3 years of clinical experience and/or experience in Emergency Medicine or Occupational Health
•       Minimum of 5 years clinical experience; 10 years preferred
•       Demonstrate ability to handle emergency situations calmly and effectively
•       Excellent patient assessment and critical thinking skills
•       Requires self-direction, leadership, delegation and teaching ability
•       Communicates tactfully and effectively with physicians, patients, families, nursing staff and other hospital departments and the public
•       Conveys a positive attitude and image internally and in the community
•       Ability to analyze and creatively solve patient care issues in a proactive and positive manner
•       Exhibits innovative ideas for nursing advancement
•       Demonstrates a professional, positive, flexible and open minded manner with a willingness and desire to go the extra mile
