Responsible for appropriately scheduling the care and services provided by Home Health clinicians to accommodate orders by physicians for referred patients. Resolves scheduling conflicts, assigns additional services and care changes accordingly, offers support to clinicians, Intake personnel, and Clinical and Care Managers.  Communicates all assignments of interdisciplinary team with all participating care team members and patient/family.

    1. Receives calls from patients, caregivers, staff, or referral sources and provides appropriate direction.

    3. Schedules home health clinicians to service patients, with input from intake personnel, clinicians, and clinical managers.

    1. Ensures all cases are staffed in accordance with physician order and patient need.

    3. Resolves patient issues related to scheduling, seeks appropriate guidance, and provides adequate follow through.

    5. Follows up with patient and caregivers with any changes made regarding new assignments or schedule alterations.

    1. Reviews and processes service referrals and assigns appropriate clinicians for visits within 24 hours of receipt.

    3. Accurately enters and maintains all staff assignments in appropriate database and correlating Excel workbooks.

    5. Maintains accuracy of clinicians’ schedules on a weekly basis.
    6. \t\t
    7. Communicates all referrals and subsequent treatment orders with intake personnel, clinicians, and clinical managers as warranted.

    9. Inputs relevant data and maintains agency census to ensure accuracy at all times.

    11. Maintains a positive working relationship with clinicians in regard to assignments, quota fulfillment, and schedule maintenance to promote uncomplicated patient care coordination.

    13. Monitors hospital data base on a daily basis to ensure timely resumption and staffing of all current patients discharged from the hospital.

    15. Provides alternate care assignments when requested changes are made by patient or caregiver; address and document all care issues, referring such to clinical managers.

    17. Assists with the reassignment of cases for staff members that are unable to meet obligations due to an emergency situation or planned time off.

    19. Prepares timely summary of patient discharge list, updates census, and communicates findings with appropriate clerical staff on a weekly basis and as warranted.

    21. Cross trains to file clerk and department assistant positions and will assume such responsibilities to best meet agency needs. Initiates assistance to other office members and accepts additional responsibilities as directed.

    23. Assists with orientation of new employees as needed.

    25. Adheres to all policies/standards as set by the Home Health Services Agency/Hospital.

    27. Adheres to all required educational/skills improvements as directed by management.

    29. Completes mandatory agency training and any additional required training as warranted.

Education and Experience:
    \t
  • Position requires High School Diploma and/or GED plus experience in the systems of a service business, specifically in scheduling or other related administrative processes.
  • \t
  • Position requires strong organizational skills, the ability to work independently and manage multiple demands simultaneously.
  • \t
  • Computer proficiency and working knowledge of Excel documents required.
