  1. Job-Specific
  • Documents a physical assessment each shift including subjective/objective data.
  • Independently identifies nursing diagnosis and develops a plan of care which addresses patient problems.
  • Communicates with patients, families and/or significant others in an effort to individualize patient's plan of care to them.
  • Communicates with physician on pertinent patient information during physician rounds or via telephone as necessary.
  • Demonstrates knowledge of order entry, utilizing the Electronic Medical Record (EMR) System.
  • Verifies that physician's orders are transcribed properly and carried out promptly. 
  • Reports significant changes in patient's condition to unit manager/charge nurse/shift supervisor and/or physician.
  • Assigns, supervises and evaluates care given by other nursing team members.
  • Coordinates multidisciplinary care, which includes patient teaching, and rehabilitation activities that are directed at patient progression toward discharge.
  • Documents nursing interventions and evaluates patient's response in the chart.
  • Takes appropriate preventative, restorative, or emergency actions that promote patient well-being using all available clinical information and adhering to physician's orders.
  • Continuously evaluates patient's responses to plan of care, teaching plans and rehabilitation activities throughout hospitalization and upon discharge.
  • Consistently adheres to hospital policies and procedures regarding isolation techniques and Barrier Precautions.
  • Assumes responsibility for attending mandatory skills validation, certifications, and continuing education requirements as designated by individual unit standards and hospital policy.
  • Assists with orientation of new employees.
  • Actively participates in hospital and unit staff meetings.
  • Observes the confidentiality of hospital information as defined per HIPPA regulations.
  • Demonstrates knowledge and skills necessary to provide care appropriate to the age of the patient served on assigned unit.
  • Accepts 'charge nurse' responsibility when directed and makes appropriate decisions according to hospital policy: collaborates with supervisory personnel as needed.
  • Implements POCT (Point of Care Testing) as documented in skills competency.
  • Participates in multidisciplinary care conference to meet patient/family goals and outcomes.
  • Maintains attendance as an essential requirement of the position as designated by hospital standards.
  • Reviews annually and adheres to all State, Federal, and local laws and standards as they relate to the ethical and legal compliance plans and policies. 
  • Performs other related duties as assigned or as workload or staffing variations require.
