  • REHABILITATION AND NURSING CARE DESCRIPTION - Hartsfield Village is a Continuing Care Retirement Community (CCRC) that celebrates the full continuum of life and promotes successful aging. Our post-acute rehabilitation program offers state-of-the-art technology and advanced rehabilitation techniques to help our patients achieve maximum function and independence. Our on-going staff education and training, and thorough knowledge in post-acute rehabilitation pathways and protocols ensure highly skilled professionals ready to serve patients with complex medical needs. The Rehabilitation and Skilled Nursing building has a total of 112 beds (16 Ortho, 45 Rehab, 51 Skilled Nursing).

    •         Assess each resident's psycho-social needs and develop a social history and goals for providing the necessary service and take part in admission process as needed. 
    •         Incorporate the social service goals with the residents' plan of care and attend care planning conferences.
    •         Assist the residents in adjusting to the facility and promote a positive environment for the continuity of relationship with family and community.
    •         Assist residents, and families to utilize the community resources when not provided directly by the facility.
    •         Maintain confidential records and interviews with residents and families as appropriate.
    •         Facilitate comprehensive discharge planning for the short-term rehabilitation patients.
    •         Perform related tasks as required.
    EDUCATION AND EXPERIENCE – Requires a Bachelor's degree in human interest field, and one year experience in a social service program for the elderly.  A person with a high school diploma or its equivalent who has or will satisfactorily complete a twenty hour or more state approved social service course and has at least two years' experience in long term care may be substituted for the degree requirement.
